Overview
Director of Social Work, plays a critical role in the lives of residents. They are responsible for all programs and record keeping related to the social services needed by residents and families, while ensuring residents’ rights are respected. The right person for this role will be an effective communicator who is compassionate, organized, and professional at all times.
Key Responsibilities
Make rounds, counsel residents, and address their psychological and social needs
Prepare and maintain resident files
Prepare face sheets for new admissions
Work with residents, families, and the Department of Social Services regarding applications, assessments, and other paperwork
Identify residents requiring additional services
Assist residents in obtaining alternate housing at discharge
Counsel residents and families regarding care level changes
Coordinate new resident orientation and help facilitate Resident Council Meetings
Protect residents’ rights, privacy, and dignity
Other tasks as requested by supervisor
Qualifications
- Bachelor’s degree or certification in social work
- Previous long-term care experience preferred
